What should the stories tell us? What si key, the solution or the process? My answer the process! If the approach is to discover the process during implementation, I'm pretty shure the project will fail. It will take longer, cost more or it's losing the user...
At the beginning is the process! If I know what to implement I can give the answer to what will it cost? what will be the timeline? who must be involved? which skill set do we need? who must be trained?
who has to do what and when with which effort? which milesstones could be set? and especially I can communicate about it. I call this a closed approach including process management, project management and most important change managemet.
you'll see that will minimize these failures. but like cobbs says: we no why projects fail, but they still fail... If we would handle our private issues like this ... would you build you own house by jsut digging a hole, buying concrete, iron, etc.. and start to work?
