Yes, that is a good question for the magic lamp (and my research team).  Let me run out and get some more AA batteries (for the lamp... my team runs on carbohydrates).  I think the answer will have to do with goal congruence, the lens with which managers view their processes, and extreme overload.  Oh, there might be a dose of incompetence too, but I need to let the light weigh in on that one.</description>
